Information We Will Need
- Verify your identity using the last four digits of your Social Security number.
- Provide a forwarding address for your final utility bill.
- Confirm the date you would like your utility service disconnected.
After your request is submitted
Once all required information is received, BPW will process the request
and schedule the service disconnection.
